Thank you for taking the On Demand Seizure Recognition and First Aid Training with Epilepsy Alliance America.
After you completed the training you should have had to complete TWO forms:
Registration Form
This form includes your contact information, your school district information and a few short evaluation questions.
Completing this form alone DOES NOT produce your Proof of Completion.
Proof of Completion
This is a short form that you much complete to generate an EMAIL that will be sent to you that will serve as you Proof of Completion.
Please print or save your email as your proof of completion, and, if required, by your school or district, submit as needed.
What happens if you completed both forms and didn’t receive an email?
If you completed the Proof of Completion and have not received your email documentation within 10-15 minutes, please check your spam and/or junk folder. If you have completed the form and the email is not in your junk folder – please complete the form again and resubmit it here
Please note that as of February 28, 2023 we have CHANGED the language for the Proof of Completion. The email you receive after the training is your Proof of Completion. There is NOT a Certificate of Completion. The email servces as your proof of completion and we removed the Epilepsy Alliance America logo from the email to facilitate printing and to avoid being trapped in spam filters. There is NO ATTACHMENT to the email.

Did you take the training on Train.org?
If you took the training on Train.org then your CERTIFICATE is issued by Train.org after you complete the Post-Assessment.
